This is the required form to sign up for the A+ Scholarship Program. All students enrolled in the A+ Tutoring class at CHS are required to submit this agreement on the first day of class.

Please see Mr. West or Mrs. Martin prior to beginning tutoring hours. All arrangements must be approved to receive credit for tutoring hours.
